  • Do I need a referral?
    Ohio is a Direct Access state, allowing you to see a Physical Therapist without needing a referral from your physician. However, we are legally obligated to notify your healthcare provider (such as your OB/GYN, MD, DO, or general practitioner) within five days of your initial evaluation. Therefore, we will ask for your general practitioner's information to comply with this requirement. *You do have the option to decline that we notify your physician. This option is define on the initial intake paperwork that you will receive once you have scheduled your evaluation.
  • Does Legend take my insurance?
    The simple answer is no. At Legend Physical Therapy and Performance, we operate as a cash-based clinic, meaning we do not contract with any insurance providers and are considered out-of-network for all insurance companies. Our decision to remain out-of-network is rooted in our commitment to providing high-quality, personalized care. Insurance companies often dictate reimbursement rates and treatment protocols, which can limit the level of care you receive. By not accepting insurance, we avoid the constraints imposed by insurance providers, allowing us to offer one-on-one care for your full time. This approach ensures that you receive in-depth analysis, individualized programming, and proactive wellness-driven treatment, helping you achieve your goals faster and reducing the overall time needed in our clinic. We offer flat rates with package pricing available for long-term conditions, providing clarity and value. While we do not guarantee reimbursement, we can provide you with a detailed report (called a superbill) after your appointment that you can submit to your insurance provider for potential reimbursement. Payment is expected at the time of service and can be made via cash, card (VISA, MC), or HSA/FSA.

  • What should I expect and what should I wear to my first appointment?
    At your first appointment, you can expect a thorough, one-hour session where we will review your medical history, discuss your main concerns, and understand your personal goals. We will conduct a detailed movement analysis and assess any contributing factors to properly diagnose your condition. After the assessment, we will create a personalized plan of care designed to help you achieve your goals, manage pain or injuries, and support your return to an active lifestyle. For your comfort, please wear loose-fitting, comfortable clothing similar to what you would wear for a workout or gym session. This will allow us to effectively assess your movement and provide the best care possible.
  • What is Dry Needling?
    Dry needling is a therapeutic technique used to treat muscle pain and dysfunction. It involves the insertion of very fine, sterile needles into specific trigger points or "knots" within the muscle tissue.   • What is your cancellation policy?
    Legend PT and Performance requires a 24 hour notice to cancel your appointment. If you cancel within 24 hours you are required to pay the fee for your appointment.
